Prophylactic treatment of factor VIII deficiency.

چکیده
S PONTANEOUS BLEEDING episodes and hemorrhage following surgery or trauma in hemophilia can be treated readily with the potent Factor VIII concentrates now available.1#{176} However, requirements for long-term prophylactic therapy to prevent hemarthroses and other spontaneous hemorrhages have not been clearly defined. Three reports of prophylactic therapy in hemophilia employing widely differing regimens have appeared: a preliminary report on the use of cryoprecipitate over a six-month period in one patient,7 an abstract on the use of a commercial Factor VIII concentrate for three months,8 and a paper describing the prophylactic use of a Factor VIII concentrate in a patient for one year.9 It is the purpose of this paper to report our experience with prophylactic treatment of four patients for periods up to two years. Two of the patients had classical hemophilia and two were twins with an unusual combination of classical hemophilia and von Willebrand’s disease. All had frequent spontaneous hemorrhages. Cryoprecipitate3 given on a convenient schedule was found to provide adequate Factor VIII to significantly decrease spontaneous hemorrhages. In one patient, the progressive enlargement of an extensive iliac pseudotumor was arrested for a two-year period. Complications of prolonged prophylactic therapy were minimal.

FACTOR V AND VIII INHIBITOR IN PATIENTS WITH COMBINED FACTOR V AND VIII DEFICIENCY
Patients with coagulation factor(s) deficiency who use coagulation therapy are susceptible to forming inhibitors against coagulation factor(s). In this survey we detected factor V and VIII inhibitor in ten patients with combined deficiency of factors V and VIII from north east of Iran (Khorassan province). It was revealed in our survey that eight patients had both factor V and factor VIII i...
متن کاملCombined Factor V and VIII Deficiency
This review summarizes current data on the pathomechanisms and new genetic findings of combined factor V and VIII deficiency (CF5F8D). Congenital haemorrhagic disorders characterized by deficiency of two clotting factors comprise an interesting group. Among dual coagulation disorders, CF5F8D is the most common type. For the first time combined factor V and VIII deficiency (F5F8D) was reported b...
متن کاملHumanistic and economic aspects of haemophilia treatment in Bulgaria. Comparison between two therapeutic approaches: prophylactic vs. on-demand treatment
The aim of the present study was to calculate the cost-effectiveness of on-demand and prophylactic treatments of severe haemophilia A for Bulgarian patients. The point of view is that of all patients suffering from severe haemophilia A. An epidemiological model was created, which includes data regarding the number of patients divided into age groups up to 74 years. In the model, the transition ...
متن کاملPost Splenectomy Fatal Pulmonary Embolism in a Patient with Moderate Hemophilia A
Hemophilia A is a bleeding disorder caused by defective production of factor VIII. The main concern associated with the disease is bleeding, especially after trauma and surgeries. Factor VIII replacement therapy is associated with substantial decrease of bleeding events during surgery. However, there have been a number of reports of thromboemblic events in this situ-ation. The present report de...
متن کاملInnovative Approaches for Immune Tolerance to Factor VIII in the Treatment of Hemophilia A
Hemophilia A (coagulation factor VIII deficiency) is a debilitating genetic disorder that is primarily treated with intravenous replacement therapy. Despite a variety of factor VIII protein formulations available, the risk of developing anti-dug antibodies ("inhibitors") remains. Overall, 20-30% of patients with severe disease develop inhibitors. Current clinical immune tolerance induction prot...
